(DOC) (PDF)
We offer Microsoft Word (DOC) and Adobe Reader (PDF) formatted files that you can use when communicating with us.
If returning your artist registration by e-mail, choose Microsoft Word (DOC). If you open one of our DOC files that asks for your input, you can type directly in the file, save it, and e-mail it back to us (emailing instructions are included on each file).
If returning your artist registration by U.S. mail, choose (PDF). If you open one of our PDF files that ask for your input, you must print out the file, provide your input and send it back to us by U.S. mail (address information is included on each file).
